Getting Started

How do I become a patient?

Start by submitting the brief inquiry form on our website.

Our physician will be in contact with you within 24 hours. The conversation will then move to HIPAA-secure messaging through Spruce to protect your privacy.

If Forge is a good fit, you’ll be directed to Practice Better to purchase your Complete Evaluation and complete intake. Once your lab draw timing is confirmed, you’ll receive a private link to schedule your physician consultation for when results are expected.

How long does the process take?

Most patients are able to begin treatment in about three weeks after completing their blood work, if treatment is appropriate.

Timing can vary depending on lab turnaround, consultation availability, and whether additional evaluation is needed.

What if treatment is not recommended after my evaluation?

The Complete Evaluation is designed to determine the right next step, not to guarantee a prescription. If treatment through Forge is not appropriate, your physician will explain why, review what your results do show, and give you a clear plan for what comes next.

Do you see patients outside Florida?

No.

Pricing & Care Model

How much does it cost to get started?

The Complete Evaluation is $449 and includes comprehensive laboratory testing and a 60-minute physician consultation.

Medication costs are separate. If treatment is started, ongoing care is managed through structured Care Blocks rather than a monthly subscription.

Is Forge a subscription?

No.

Forge is not a subscription clinic. You pay for physician visits, lab testing, medication costs, and follow-up care as needed.

There are no monthly membership fees just to stay active, no call-center middlemen, and no generic refill model replacing physician-led care.

What is a Care Block?

A Care Block provides ongoing physician management: one physician follow-up visit, review and interpretation of follow-up laboratory results, treatment adjustments, direct messaging with your physician throughout the block, and medication and refill management.

Your first Care Block is billed when your initial prescription is sent, after you and your physician decide to begin treatment. It is not charged when you purchase your evaluation.

Care Blocks are $299 each. Each of the first two blocks covers three months, with planned checkpoints to assess your response and work toward a stable plan. As your plan becomes established, blocks may extend to six months at the same price.

Laboratory testing and medications are billed separately.

How much do follow-up labs cost?

Follow-up panels typically cost $69–$129. Your panel is selected based on your treatment and monitoring needs. Additional testing may cost more, and costs are reviewed before testing.

Are labs included with the Annual Review?

No. The Annual Review is a 60-minute physician consultation, held once a year to look beyond your current treatment plan and consider your updated lab work, how you feel, and the concerns or goals that matter to you now.

The consultation is $229, billed separately from Care Blocks. Laboratory testing is also separate, and costs are reviewed before testing.

What if I have concerns outside my current treatment plan?

Care Blocks cover questions and adjustments related to your established plan of care.

New or more complex concerns may require a separate focused visit, with a cost of $99 for an approximately 20-minute visit.

Are medication costs included?

No.

Medication costs are separate from visit and lab fees.

All medications have a $10 shipping fee per shipment.

If medication is part of your plan, your physician will review the cost, dosing, and pharmacy options before treatment starts.

Typical estimated costs:

  • Testosterone therapy: $40 to $120 per month
  • Injectable GLP-1 therapy: $200–$330 per four-week supply. Higher doses may cost more.

Combination protocols, high-dose GLP-1 therapy, adjunct medications, fertility-support medications, and targeted supplements are priced separately and discussed before treatment starts.

What does the GLP-1 medication estimate cover?

The $200–$330 estimate per four-week supply applies to injectable GLP-1 medication prepared by a compounding pharmacy. Cost depends on the medication and prescribed dose; higher doses may cost more. Forge can also prescribe brand-name GLP-1 medications when clinically appropriate, but pricing and availability cannot be guaranteed. Forge does not submit or manage insurance prior authorizations. Compounded medications are not FDA-approved or generic versions of FDA-approved medications.

Do you accept insurance?

Forge is a direct-pay practice. We recommend our flat-rate cash lab pricing so you know the cost upfront.

If insurance denies coverage, the lab’s billed charges may be higher than Forge’s cash price. Deductibles and uncovered portions can also make insurance-billed testing more expensive.

If you prefer to use insurance for labs, Forge can adjust your evaluation price and provide the appropriate orders and diagnosis codes. Coverage is not guaranteed, and Forge does not handle insurance approvals or appeals.

Labs

Where do I get my blood work done?

You’ll complete your blood work at a Quest Diagnostics location near you. Forge will provide your lab order and instructions before you go.

What’s included in the comprehensive lab panel?

The Forge baseline panel includes:

This panel covers the major drivers of hormonal, metabolic, and overall health concerns in men. Additional targeted testing may be added if your symptoms require deeper investigation.

Why does Forge use the highest-standard methods available for testosterone and estradiol testing?

Because weak hormone testing can lead to weak decisions.

For these key hormone tests, Forge uses the highest-standard methods available: Free Testosterone by equilibrium dialysis and sensitive estradiol LC-MS/MS—not calculated estimates or older assay methods.

That matters because borderline hormone decisions can change when the wrong method is used. A man can be told everything looks “normal” when the better test tells a different story.

Forge uses better testing so your plan is not built on shortcuts.

Can I use recent outside labs?

Recent outside labs may qualify for a discount on your initial evaluation if they meet Forge’s testing requirements and were drawn within the past two months. Results are reviewed individually, and any discount reflects the cost of the accepted tests. Missing or unsuitable testing will still need to be completed.

Forge Difference

How is Forge different from my primary care doctor, urologist, or an online clinic?

Primary care is usually built for broad medical care, not detailed hormone and metabolic management.

Urology is important for many men’s health problems, but not every urology visit is built around testosterone, weight, energy, sleep, libido, metabolic health, and long-term treatment adjustment.

Online clinics often go too far the other way: quick labs, quick prescription, weak follow-up.

Forge was built around what these models often miss: deeper evaluation, better hormone testing, individualized treatment, and physician-led follow-through.

What if my labs are “normal” but I still feel terrible?

“Normal” does not always mean optimized, explained, or fixed.

Symptoms can come from patterns that basic labs miss or underweight, including low Free Testosterone, insulin resistance, estradiol issues, poor sleep, medication effects, weight gain, or poor recovery.

The goal is not to force treatment.

The goal is to understand what is actually going on and build a plan that makes sense.

What if I’m already on TRT, GLP-1 medication, or another men’s health treatment and want to transfer to Forge?

Forge can evaluate patients who are already on treatment and want physician-led management.

You’ll still need an appropriate review of your current regimen, recent labs, symptoms, goals, safety markers, and medication history.

If your current treatment plan is appropriate, Forge may continue or adjust it. If it is not, your physician will explain the concerns and recommend a safer plan.

For testosterone therapy, Forge must be your sole managing provider while you are under our care.

Medications

Does Forge offer testosterone therapy?

Yes, when clinically appropriate.

Treatment may include injectable, topical, or oral testosterone. The choice depends on your symptoms, repeat hormone testing, medical history, fertility goals, risks, cost, and preference.

Testosterone is not prescribed from one number alone. Your physician reviews the broader hormonal and metabolic picture, explains the options, and builds monitoring into the plan.

What if TRT isn’t right for me?

If TRT isn’t right for your situation, you’ll hear that from us before you’re prescribed something. Our care model isn’t built on keeping you locked in a subscription for medication. There are multiple effective treatment paths for low energy, weight gain, and metabolic concerns, and the right one might not involve testosterone itself at all.

What if future fertility matters to me?

Testosterone therapy can suppress sperm production. Forge strongly encourages baseline semen analysis and consideration of sperm banking before starting treatment—especially if having children remains a possibility.

If you are trying to conceive now or in the near future, tell us before treatment begins. Forge can evaluate your hormone concerns and discuss options that account for your fertility goals, including specialist involvement when appropriate.

Even if you do not currently plan to have children, we recommend considering testing and sperm banking before testosterone therapy because plans can change.

What other treatments may Forge consider?

Treatment is built around what is driving your symptoms, not around one medication.

Depending on your evaluation, your plan may address hormone balance, metabolic health, weight, sexual performance, sleep, recovery, or related contributors. Options may include prescription treatment, evidence-based supplements, lifestyle strategy, additional testing, or referral when a concern falls outside Forge’s scope.

Your physician will explain what is being recommended, why it fits, what it costs, and how it will be monitored before treatment begins.

Does Forge prescribe GLP-1 medications?

Yes, when clinically appropriate.

Forge may use GLP-1 or dual-incretin medications such as semaglutide or tirzepatide as part of a broader medical weight loss and metabolic health plan.

Medication is not the entire plan. Forge also evaluates insulin resistance, hormones, sleep, activity, nutrition, prior treatment attempts, and long-term sustainability.

Do I need to qualify for GLP-1 the way insurance requires?

No.

Forge is a direct-pay medical practice, so your treatment plan is not based on your insurance company’s coverage rules.

That does not mean GLP-1 medication is automatic. Your physician still reviews your health history, goals, risks, labs, prior treatment attempts, and whether medication is medically appropriate.

Can I do GLP-1 and TRT at the same time?

Yes, when clinically appropriate.

Hormonal health and metabolic health often overlap. Some men need testosterone-focused care, some need weight loss or insulin resistance treatment, and some need both addressed together.

Forge coordinates those decisions instead of treating them like disconnected problems.

Safety

How is treatment monitored for safety?

Your physician reviews your medical history, current medications and supplements, and baseline labs before recommending treatment.

Follow-up visits and lab testing are selected for your treatment and risk factors. Your response, side effects, and safety markers guide decisions about whether to continue, adjust, or stop treatment.

Tell your physician about new diagnoses, medications, or supplements so your plan can be reassessed when needed.

What should I do if I notice side effects?

For non-urgent side effects or new symptoms, contact your physician through Spruce. Include the medication and dose you are taking, what you are experiencing, and when it started. Your physician will review the concern and advise on next steps.

Severe or rapidly worsening symptoms need prompt in-person assessment. For a medical emergency, call 911 or go to the nearest emergency department. Do not wait for a message response.

Can I use Forge for urgent medical problems?

No.

Forge Men’s Health does not provide emergency care or urgent medical care.

If you are experiencing chest pain, severe shortness of breath, fainting, stroke-like symptoms, severe allergic reaction, suicidal thoughts, or another medical emergency, call 911 or go to the nearest emergency department.

Forge is for appropriate virtual men’s health care, not emergencies.
