Testosterone optimization

Look beyond the reference range.
Low testosterone doesn’t show up as a number. It shows up as a shorter fuse, workouts that feel harder, recovery that takes longer, and a fading drive. Treatment should give those back.
A higher number is not the goal. The score that matters is how you feel and function.
And the number alone can’t tell you much. One man can feel better at a lower total testosterone than another does at a higher one. Carrier proteins like SHBG change how much testosterone is actually available to your body, which is why free testosterone matters. But free testosterone is often calculated from a formula rather than measured, and the estimate can miss right where borderline decisions are made. Forge measures it directly, by equilibrium dialysis.
A normal total testosterone result doesn’t tell the whole story. A low result doesn’t prove it explains your symptoms.
So the result is weighed against your symptoms, lifestyle, metabolic health, and medical history.
Treatment options

There is no universal protocol. The medication, dose, route, timing, and supporting treatment all shift with your physiology, your response, and your goals.
Injectable Testosterone
The most precise control over dose and timing, and the most adjustable when your response needs fine-tuning.
Topical Cream
A daily, needle-free option that produces a different balance of testosterone, DHT, and estradiol and suits some men better.
Oral Testosterone
Newer pill-based options when injections or cream do not fit. They offer convenience, but less control over dosing and levels than injectable testosterone.
hCG and Enclomiphene
Used to preserve fertility or maintain your own natural production, alone or alongside treatment.
The protocol adapts to you—not the other way around.

Fertility Planning
Current and future fertility goals are addressed before hormone treatment begins, with semen testing used when preserving sperm production is part of the plan.
Hair Preservation
Hair priorities are considered when choosing and adjusting androgen treatment, with targeted treatment available when needed.
Ongoing safety
Safety doesn’t end when the prescription is written.
Forge follows blood pressure, hematocrit, PSA and prostate health, sleep-apnea risk, and how you’re actually responding. When something needs to change, the dose, route, timing, or supporting medication changes with it. Testosterone is actively managed, not refilled on autopilot.
Common Questions
How does Forge handle estradiol control?
Men need estradiol for sexual function, bone health, and cardiovascular health. The goal is not to drive it as low as possible.
Estradiol management starts with the testosterone protocol itself. Dose, delivery method, injection frequency, hCG use, and body composition can all shift that balance.
When adjusting those levers is not enough, an aromatase inhibitor is another available tool—not a medication attached to every testosterone prescription.
Is testosterone therapy lifelong?
Often long-term, but not automatically lifelong.
Testosterone therapy replaces what your body isn’t producing in sufficient amounts. If the underlying deficiency persists, ongoing treatment is usually needed to maintain the benefits. If a reversible cause improves, such as excess weight, untreated sleep apnea, or a medication effect, we reassess whether treatment is still needed.
TRT also suppresses your own production, so stopping should be planned, not abrupt. Recovery varies, and symptoms can return. We discuss the likely commitment before you start and revisit it as part of your ongoing care, including each Annual Review.

You’ll still need an appropriate review of your current regimen, recent labs, symptoms, goals, safety markers, and medication history.
If your current treatment plan is appropriate, Forge may continue or adjust it. If it is not, your physician will explain the concerns and recommend a safer plan.
For testosterone therapy, Forge must be your sole managing provider while you are under our care.
